Eligibility Requirements
To qualify for our transportation services, you must:

Be able to walk independently or with the aid of a walker.

Be 60 years of age or older.

Not be on Medical Assistance.
-
Be unable to drive yourself, without a caregiver who can transport you, and not qualify for other transportation programs.

Require transportation to a medical or essential appointment in the local area.

Scheduling Your Ride
While we strive strive to provide transportation to all who need it, we cannot guarantee the availability of drivers in your area. We recommend having a backup plan for transportation in case a volunteer is unavailable. To ensure you receive the service you need, please follow these steps:
1. Call Age Well Arrowhead
Call Age Well Arrowhead at least 5-7 business days before your scheduled appointment to arrange transportation.
2. Share Specific Appointment Details
Inform our Volunteer Coordinator about your transportation needs like date, time, and pick-up/drop-off location.
3. Be Ready On The Day Of Your Appointment
On the day of your appointment, your driver will pick you up, take you to your appointment, and wait to bring you back home.
